Allied Solutions Center for the Performing Arts Transportation Grant

Final Request for Funds

 

Thank you for attending an event at Allied Solutions Center for the Performing Arts. In order to receive your grant payment from Allied Solutions Center, please complete and submit the information requested below.

 

The funds requested must be actual transportation-related expenses for a field trip to Allied Solutions Center for the Performing Arts. The field trip must have already occurred. Only those who have had a Transportation Grant request approved by Allied Solutions Center may submit a final request for funds.

 

Requests must be received within 45 days after the performance attended.

The name must match the organization/individual listed on the W-9 which you will also submit.
Note: Schools could apply for grants up to $250 per bus. The maximum grant per school/per event is $600. The reimbursement amount may not exceed the amount approved by the Center in your original Transportation Grant Request.

The name on the W-9 must match the name to whom Allied Solutions Center will make the check. If you do not have a completed W-9, please download this blank W-9 to complete. Please make sure the W-9 is signed and dated.
